Unusual case of cecal volvulus in a pediatric patient. Operative note describes redundant cecum which had volvulized 3 turns. This likely occurred due to congenital lack of peritoneal fixation of the cecum, resulting in increased mobility. A few "flimsy adhesions" were noted in the operative note which may have served as a fulcrum for rotation. Cecal volvulus accounts for approximately 1 % of the intestinal obstruction. Etiology includes congenital development of fixation, secondary to adhesion, scarring or mass lesions.

2020-07-24 · Cecal volvulus may be organoaxial (true cecal or cecocolic volvulus) or mesentericoaxial (cecal bascule). The former involves the distal ileum and ascending colon twisting around each other, in much the same way as a sigmoid volvulus.

We reviewed the clinical histories of 11 patients diagnosed with cecal volvulus at endoscopy or surgery. All CT findings were significantly more common in patients with cecal volvulus (p < 0.01) other than mesenteric engorgement for reader 1 (p = 0.332).

The large loop of dilated bowel in the central abdomen is the volved cecum, with a resultant small bowel obstruction. This film is made more challenging by the gas retained in the semi-dilated ascending colon. At surgery a cecal volvulus was id CT demonstrates the large bowel mass is a grossly distended food-filled cecum that is not in the right iliac fossa but in the left side of the abdomen.
